What do you think of the role of CCBs and the management of hypertension in the Asian population? Based on your experience， what is the current status of CCBs like amlodipine in the anti-hypertensive field in Korea?

    Dr. Park: CCBs are the most predominant anti-hypertensive drug used in Korea and especially amlodipine is the best-seller. It is the number one anti-hypertensive drug.  In  East Asian countries such as Korea， Japan and I think China too， there are two important factors. One factor is the disease population. Western people have more coronary artery disease but Asian people have more stroke，  –  stroke is the number one cause of death. So CCBs are more protective for stroke.  The other factor is that in my country， Korea， we have a very high salt intake and CCBs are  very good for the hypertensive patient with a high salt intake. CCBs have enough of an anti-hypertensive effect even though they take in a lot of salt， but other drugs such as ACE inhibitors or ARBs don’t affect or more significantly decrease hypertension in the presence of high dietary salt. This is why CCBs are a very important anti-hypertension medication for Asian people.

    Dr. Park：CCB是韩国使用最广泛的降压药物，尤其是氨氯地平，它是销量第一位的降压药物。对于东亚人群（例如韩国、日本、中国）来说，有两个重要因素需要考虑。一个是疾病人群。西方人群的冠心病发病率更高，而亚洲人群的卒中发病率更高。卒中是首位死亡原因。在预防卒中方面，CCB的获益更多。另一个因素是，在韩国，我们的盐摄入水平很高。CCB对于高盐摄入的高血压患者非常有益。即使患者的盐摄入量很高，CCB仍然能够充分控制血压。但是其他类降压药物，例如ACEI或ARB，对于高盐摄入的高血压患者，降压疗效则不够充分。上述两点说明CCB对于亚洲人群来说是非常重要的降压药物。

    International Circulation: In recent years， many guidelines have pointed out that the five classes of anti-hypertensive drugs can be used as first line drugs and the choice should be made based on risk factors such as target organ damage and established cardiovascular disease. What kinds of patients are most appropriate for amlodipine treatment?

    国际循环：近年来，众多指南提出5大类降压药物均可作为一线降压药物，药物的选择应当基于危险因素，靶器官损伤和伴随的心血管疾病。CCB（氨氯地平）最适宜的人群包括哪些？哪些患者应该优先选择CCB，例如氨氯地平？

    Dr. Park: In my opinion， amlodipine has a very good anti-atherosclerotic effect and a vasodilatory effect and increases nitric oxide and it also has an antioxidant effect. For preventing full-blown cardiovascular disease such as stroke， heart attack and kidney disease， the antioxidant effect and increasing nitric oxide are very important. There are many clinical trials proving its multiple effects in preventing stroke， coronary heart disease and other target organ damage. So amlodipine has approval for coronary heart disease other than hypertension.

    Dr. Park：我认为氨氯地平有很好的抗动脉粥样硬化的作用，血管扩张作用以及升高NO水平的作用，同时它还具备抗氧化作用。对于全面预防心血管疾病，例如卒中、心衰和肾病而言，抗氧化作用和升高NO水平都是至关重要的。许多临床研究证实了氨氯地平在预防卒中、冠心病和其他靶器官损伤方面的多重获益。因此，除了高血压之外，氨氯地平还获准用于治疗冠心病。
