编者按:CIT发展13年来,见证了中国介入的飞速发展。在CIT大会共同主席、美国哥伦比亚大学Martin Leon教授看来,这已成为CIT和TCT持续合作发展的重要契机。在CIT 2015现场,《国际循环》采访到大会共同主席Martin Leon教授,他对CIT和中国介入发展给出了充分肯定。
International Circulation:You have been the co-chair of CIT for many years, what’s your opinion about the progress of China’s PCI in these years? And what do you think about the most exciting features of this meeting?
《国际循环》:您已担任CIT大会共同主席多年,您目睹CIT在国内的成长和对世界影响的变化,对中国这些年介入的发展有哪些看法?尤其您认为今年CIT最大的亮点是什么?
President Leon : Well certainly China PCI has dramatically evolved over the past decade since we began working with CIT. The physician training has been outstanding, the quality of the angioplasty continues to evolve, and even though many years ago we were doing a great deal of training, now the operators have such experience that they have become close collaborators on many clinical research projects. I have to say, with 480,000 PCI being done in China last year that they have become one of the leaders in PCI globally. This is very important to us because now we have a partner, a partner in collaboration, a partner here in research, and a partner in education. This year CIT is moving in several different directions, certainly focusing on complex PCI, things like chronic total occlusions, focusing on different interventional procedures like structural heart procedures, especially transcatheter aortic valve replacement and also focusing on intravascular imaging and physiology which is some of the major trends right now in intervention around the world.
Martin Leon:随着CIT的发展壮大,中国介入心脏病学也取得了很多进展。医生培训很出色,血管成形术质量持续改进,尽管多年前我们做了大量培训,现在很多术者经验丰富,可以在多个临床研究项目中成为我们亲密的合作伙伴。去年中国的PCI总量达48万例,是全球介入数量最多的国家之一。这对我们成为合作伙伴至关重要,我们可以共同研究、学习和探讨。今年CIT关注很多不同的方面,关注复杂PCI如慢性完全闭塞病变,关注不同介入术式如结构性心脏病手术,特别是经导管主动脉瓣置换术,也关注血管内造影和生理学,这也是未来世界关注介入医疗的主要趋势。
International Circulation:You will report the issue concerning TAVR and TMVR(Transcatheter Mitral Valve Replacement)Therapies, what’s the take-home message for Chinese physicians?
《国际循环》: 您在会上将对TAVR和经导管二尖瓣置换的临床实践作相关报告,对中国医生来说,您认为对于这两种术式的关键信息是什么?
President Leon: I think it is very important because aortic stenosis is a disease of aging, the only real treatment is surgery and knowing the culture of the Chinese people, if we can avoid surgery, we should try to avoid surgery. I think that China right now is behind in transcatheter treatment of aortic valve disease and there is a dramatic evolution which is occurring around the world with many different new devices, particularly the SAPIEN 3 and the Evolute R. There is no question that China needs to catch up, get more training, and to generalize the use of transcatheter valve procedures to manage these complex patients with aortic valve disease.
Martin Leon:我认为这很重要,因为主动脉瓣狭窄是老年患者常见病,中国文化认为唯一有效的治疗就是手术。但我们可以尽量避免手术。中国目前大力发展经导管瓣膜治疗,该术式在世界范围内进步迅速,出现了很多新型器械,尤其是SAPIEN 3和Evolute R。当然,中国也需要赶上前沿,进行更多培训,实现经导管瓣膜治疗的更新换代,以便管理这些复杂的主动脉瓣膜病患者。
