ICIRCULATION: Just now， I watched the live of operation， what do you think of the Chinese Interventional Technology?

Prof. E. Grube: I think it’s an outstanding progress that I have seen in CIT 2008， I’ve been following the CIT for a number of years， and I think the state of art and skill and knowledge is outstanding and one of the examples how the populations and positions are catching up the actual standard of the world， and I’m impressed how fast it goes， and the quality of work is very very good in China.

ICIRCULATION: Would you share your experience about the PCI with us?

Prof. E. Grube: I’ve been doing the work in full time for four years now， and the work that’s been done here is like， in every other place in the world: high quality， very professional and very successful， and you’re using the technology that are being used outside the world， outside china， exact the same way. So if we change our world now: we come from United States to China; we will do the work in the same way. The truth is that the same questions that are being asked， it is the same high level of expertise here， it’s proud of that China shares this as a member of faculty.

ICIRCULATION: With many clinical trials， how do you evaluate the DES safety?

Prof. E. Grube: Well， the DES safety obviously is a big question， a very big question， very big question mark， and I think we have to be very careful on how we look into the future with this technology; one way of looking at it is， at the stent， at the polymer， and at the drug. The stent does not probably affect very much the safety， but it affects the performance; the drug， we know that we will change this in the future. The polymer is the question that is open. And I think in order for the true second generation drug eluting stent， I think the bio-absorbable polymer， is true next step into the safety， into the better safety， it is because the polymer go away， and the drug will not be used any more， and I think that adds safety， even better， hopefully in the near future， we will see a drug stent with an outstanding polymer， but this is going to out a little bit， if we talk about safety， we have to talk about the polymer and the bio-absorbable stent， and in China and in Europe now is definitely a step in the right direction and better safety.

ICIRCULATION: Please introduce your clinical experience about the therapy strategies of CTO.

Prof. E. Grube: Usually we will justify the reopening of vessel; we need to know the tissue that dependent of the artery is viable. It’s not dilating or opening the vessel that doesn’t supply to the living muscle， so we would like to see the ischemia by the muscle in the area of the artery. I usually use guide wire to start， and then I use the either intermediate wire or Miracle 3 gram or 6 gram. And if that does not have the success， I will choose Conquest Pro， then usually we are successful， I do not like hydrophilic wire. Hydrophilic wire is hydrophilic coating， you know， because I don’t have the right sense for it. My choose is intermediate or Miracle 3-6， and Conquest Pro， I usually use ante grade， I do not go retrograde， and I like two-wire approach if I’m going into the vessel， I need one wire in it， and then try to find the rightlumen.
