<International Circulation>:The International Stroke Conference (ISC) is the world’s largest meeting dedicated to the science and treatment of cerebrovascular disease. As chair of ISC 2013, could please comment on what your hopes are for this year’s meeting? What do you think are the hot topics this year?

Prof.Greenberg: This year’s ISC, like every year’s conference will really be driven by the scientific sessions presented. The scientific sessions will be presented in multiple forms such as posters, oral abstracts and invited sessions such as pre-conference symposiums covering particularly active or novel areas related to stroke. There will also be presentations on late-breaking usually clinical trials related to stroke. These trials are often the feature of the meeting and get the most publicity. There is a wide range of types of scientific abstracts that will be presented but just to highlight a few there will be the usual group of recently breaking clinical trials. Notable among those will be two major trials of intra-arterial endovascular therapy for acute stroke. There will be the MR RESCUE (The Mechanical Retrieval and REcanalization of Stroke Clots Using Embolectomy) study and the IMS Ⅲ (Interventional Management of Stroke III Trial) study both of which will be presented for the first time publically at ISC. In this really active and quickly changing field and there is the question of when to choose intra-arterial and endovascular therapy for acute stroke as opposed to already established therapies, like intra-venous tissue plasminogen activator (tPA) for acute stroke. The conference will also include another much-awaited trial not in acute stroke therapy but in prevention is the SPS3 (Secondary Prevention of Small Subcortical Strokes Trial) study which is the trial on the prevention of recurrence small strokes which we know are important contributor to long-term disability and vascular cognitive impairment. This study has been done in two arms. One of the arms has already been reported as not being effective which was the addition of a second antiplatelet treatment, clopidogrel on top of aspirin. The result presented at this meeting will address the other arm which involves a very tight blood pressure control as opposed to more lose, conventional blood pressure control and the other trial presented as well.

Also, for the first time, this year we will have a rapid-fire of shot presentations related to possible future stroke developments: “the next best thing in stroke at lightning speed.” We have asked eleven experts asked to give five minute speeches rather than our usually talk being driven primarily data and very careful descriptions of what has been done. Instead we will turn, not to science fiction, but to realistic views of what the future might hold. We are using the rapid-fire format to have a little fun but also to stimulate people to think in a forward-looking way and to stimulate discussion among the audience. So, this will be the first time we have used this format at the International Stroke Conference and we are looking forward to seeing how it does with engaging the audience.

One other unique feature of this year’s meeting will be a special contribution to C. Miller Fisher. Dr. Fisher would have turned 100 in 2013 but passed away in April of 2012 and there will be several tributes to Dr. Fisher. Although there have been many great contributors to the stroke field, Dr. C. Miller Fisher was a once in a generation giant as he has made an enormous contribution to our modern understanding of stroke. One of the symposiums will highlight the way Dr. Miller Fisher’s contribution continues to influence the direction of stroke research in multiple areas.
